A complete application should include the following:

1. Complete form ISBE 73-03D Application for School Service Personnel/ Administrative. The Illinois college or university must complete the section verifying that the applicant completed a state approved program, i.e., affix the institutional seal and signature of the college or university certification officer.

2. Complete form ISBE 73-10 Certification Registration Form.

3. Official transcripts from all colleges attended in original sealed envelope. .
4. A $30 fee is required. Checks are made payable to ROE #40. Please call for additional registration fees.
