For Out-of-State applicants, holding no prior Illinois Certificate:
1.Complete form ISBE 73-03E Application for Master Certificate
2. Complete form ISBE 73-63 Request for Issuance of Provisional Certificate
(For individuals who hold a valid comparable certificate from another state only.) A copy of the front and back of the out-of-state certificate must accompany this request.
3. Complete form ISBE 73-05 State Approved Program Verification
(For individuals who have completed a state approved teacher education program for the certification area requested.)
This form needs to be sent directly to the Illinois State Board of Education at:

4. Official transcripts from all schools attended in original sealed envelope. Previously filed transcripts do not have to be resubmitted. The Illinois State Board of Education will notify the applicant if resubmission of older transcripts will be necessary.
5. Photocopy (front and back) of out-of-state certificate, if applicable.
6. Employment Verification: If you have teaching experience, please submit an original letter on agency letterhead (no copies) stating all employment dates, the school in which you were employed, your teaching assignment and the type of certificate you held. This letter will only be accepted if signed by a district superintendent, assistant superintendent, special education director or non-public school administrator.
7.Photocopy of NBPTS certificate.
8. Please call for additional fees for registration.
For applicants holding a prior Illinois certificate:

3. Photocopy of NBPTS certificate.
