A complete application should include the following:
1. Complete form ISBE 73-71 Certification Update/Duplicate Certificate Request 
Please complete the following option(s) that apply to you:

• Section I-Notice of change of address
• Section II- Name change
• Section III-A degree change (attach an official transcript in the original sealed envelope to update our records)
• Section IV-Request for a copy of your certificate(s).

2.  Pay the applicable fee amount
3. Sign and date the form and return to ROE #40.
