NOTIFICATION OF CALIBRATION CHECK CLINIC

Please plan to bring your audiometer(s) to the calibration check clinic indicated below. No need to fill out forms ahead of time; they will be done at the time of the calibration check.

​All audiometers should be clearly marked on the outside with a tag or label as to the agency/school district the audiometer belongs to, including a contact name & phone number. This will aid in pick-up to assure all audiometers get returned to the proper person.

Drop off & pick-up information:
The building is open 8:00- 4:00. You may drop off audiometers between 8:00 a.m. –  3:30 p.m. on Monday March 28, 2022, OR between 8:00 – 9:00 a.m., the day of the clinic, TUESDAY March 29, 2022.

Please pick up audiometers between 1:00 p.m. & 4:00 p.m. On TUESDAY, March 29, 2022 or anytime between 8-4 the rest of the week. 
           
Calibration check clinic will begin about 9:00 am.

PUT A NOTE ON YOUR MACHINE IF YOU ARE WAITING WITH A NAME & NUMBER TO BE CALLED AFTERWARDS
 
The fee is $ 30.00 per audiometer & an invoice will be emailed to this email address after the calibration check clinic is completed unless an alternative email was listed.
